Cefradine

Description:
Sicef contains Cefradine which is a semi-synthetic first generation cephalosporin antibiotic that works in a similar way to Penicillin by inhibiting bacterial cell wall synthesis.

Presentation
Sicef ® 250 Capsule: Each capsule contains Cefradine BP 250 mg.
Sicef ® 500 Capsule: Each capsule contains Cefradine BP 500 mg.
Sicef ® Pediatric Drops: When reconstituted each 1.25 ml contains Cefradine BP 125 mg.
Sicef ® Powder for suspension: When reconstituted each 5 ml contains Cefradine BP 125 mg.
Sicef ® DS Powder for suspension: When reconstituted each 5 ml contains Cefradine BP 250 mg.

Indications
Upper respiratory tract infections, ENT infections, Lower respiratory tract infections, Urinary tract infections, Skin and soft tissue infections, Gastro-intestinal tract infections, Infections in the bones and joints, Septicaemia, Endocarditis and Prophylaxis of postoperative infections.
Dosage and Administration
Adults – Oral: The usual dosage range is 1-2 gm daily in two or four equally divided doses. Severe or chronic infections may require larger doses up to 4 gm daily in divided doses or as advised by the physician
Specific dosage recommendations:
Respiratory Tract Infections – 250 mg to 500 mg four times daily or 500 mg to 1 g twice daily.
Urinary Tract Infections – 500 mg four times daily or 1 g twice daily.Skin and Soft Tissue Infections – 250 mg four times daily or 500 mg to 1 g twice daily. Gastro-intestinal Tract Infections – 500 mg three to four times daily. Children – Oral: The usual dose is 25 to 50 mg/kg/day, given in two or four equally divided doses. For Otitis Media daily doses from 75 to 100 mg/kg in divided doses every 6-12 hours are recommended. The oral dosages should be taken before meals. Elderly: There are no specific dosage recommendations or precautions for use in the elderly except, as with other drugs, to monitor those patients with impaired renal or hepatic functions. Larger oral doses (up to 1 gm four times daily) may be given to all patients, irrespective of age and weight, for chronic infections or depending on the severity or site of infections.

Contra-indication, side effects and warnings etc.

Contraindication
Known hypersensitivity to Cephalosporin.

Side-effects
Rarely, Cefradine may induce a hypersensitivity reaction and gastrointestinal disturbance which induces nausea, vomiting, diarrhea, glossitis, heartburn, dizziness, tightness in the chest, vaginitis, candidal overgrowth, urticaria, skin rashes, joint pains, edema. Mild transient eosinophilia, leucopenia and neutropenia, positive direct Coombs test and pseudomembranous colitis have been reported.

Use in pregnancy and lactation
Although animal study has not shown any teratogenicity, safety in pregnancy has not been established. It is present in breast milk and should be used with caution in lactating mothers.

Drug interactions
Bacteriostatic antibiotics may reduce cephalosporin effectiveness. Probenecid may increase and prolong cephalosporin plasma levels and toxicity.

Precautions
Known hypersensitivity to penicillin.
Pharmaceutical precautions
Powder for suspension: Store in a cool and dry place. After reconstitution use within 14 days if stored in a refrigerator, or 7 days at room temperature.

Sicef ® 500 Capsule: Box containing 20 capsules in Alu-Alu Blister Pack.
Sicef ® Pediatric Drops: Bottle containing powder to make 15 ml suspension. When reconstituted each 1.25 ml contains Cefradine BP 125 mg.
Sicef ® Powder for suspension: Bottles containing powder for suspension of 100 ml, when reconstituted each 5 ml contains Cefradine BP 125 mg.
Sicef ® DS Powder for suspension: Bottles containing powder for suspension of 60 ml & 100 ml, when reconstituted each 5 ml contains Cefradine BP 250 mg.
